Community Participation

Community Participation is about helping you stay connected, active, and involved with the world around you. At Reach Assist, we support you to join activities that match your interests — whether that’s sport, art, volunteering, social events, or community programs. 

We’re there to offer encouragement and practical support so that getting involved feels exciting, not overwhelming. Being part of the community isn’t just about attending events — it’s about building friendships, finding purpose, and experiencing new opportunities. Innovative Community Participation

Sometimes, a different approach is what’s needed to help build confidence and connection. Our Innovative Community Participation services offer flexible, creative options to help you try new activities, build skills, and connect with people in ways that are tailored to your interests and strengths. Group and Centre Based Activities

Being part of a group can open up opportunities to learn new skills, make friends, and build social confidence. Our Group and Centre Based Activities provide structured, supportive environments where you can take part in meaningful activities alongside others.

Travel and Transport Arrangements

Getting where you need to go shouldn’t be stressful or complicated. Our Travel and Transport support helps you attend appointments, school, work, community events, or social outings with ease. At Reach Assist, we focus on making travel simple, safe, and reliable, whether that means providing direct transport or supporting you to use public transport independently. 

Having access to consistent travel support can make a big difference to your independence, opening up more opportunities for connection, learning, and community involvement. We work around your schedule and needs, making sure travel is one less thing for you to worry about.

Supported Independent Living

Supported Independent Living (SIL) is designed to help you live as independently as possible while still having access to the right support when you need it. At Reach Assist, we offer SIL services tailored to your individual needs, whether that means assistance with daily tasks, overnight care, or building your confidence in a shared or individual living arrangement. 

Our experienced team works closely with you to create a living environment that feels safe, comfortable, and truly yours. We’re there to encourage your independence, not take it away. Through our Supported Independent Living services, we help you build routines, connect with your community, and achieve your personal goals while always respecting your preferences and choices.

Community Nursing Care

Community Nursing Care provides essential medical support to help you manage your health at home and in the community. Our qualified nurses at Reach Assist deliver a range of clinical services, from wound care and medication management to chronic disease monitoring and complex care needs. We work closely with you and your healthcare team to make sure your support is thorough, personalised, and responsive to any changes in your condition. 

Having access to skilled nursing care means you can maintain your independence without compromising your health. With our community nursing services, you can feel confident knowing that professional support is just a call away, and your care is handled by people who genuinely understand your needs.

Daily Personal Activities

Support with daily personal activities is about helping you maintain your dignity, independence, and comfort with everyday tasks. Reach Assist offers practical assistance with personal care, including showering, dressing, grooming, toileting, and eating, always with sensitivity to your preferences and routines. 

Our support is flexible — whether you need assistance a few times a week or daily help to start and end your day. We know how important it is to feel in control of your life, so we focus on working with you, not taking over. By assisting with daily personal activities, we help you stay active and independent while giving you the confidence that the right support is there when you need it.

High Intensity Daily Personal Activities

High Intensity Daily Personal Activities involve supporting participants with more complex or intensive needs. At Reach Assist, we have experienced, qualified staff who are trained to provide safe, high-level support for tasks such as complex bowel care, catheter management, PEG feeding, ventilator support, and more. 

We follow individual care plans carefully, working alongside healthcare providers to ensure the highest standard of support is delivered consistently. Our approach is not just about meeting medical needs — it’s about treating every person with dignity, respect, and individual care. We recognise that high-intensity needs require more than just technical skill — they require trust, patience, and a focus on the whole person, not just their medical requirements.

Household Tasks

Support with household tasks can make a big difference to your quality of life. At Reach Assist, we’re here to help you stay comfortable, safe, and independent at home by assisting with domestic duties that might have become difficult. 

Our household task support covers cleaning, laundry, dishwashing, changing bed linen, and other everyday jobs that help maintain a healthy living environment. We work with you to understand what’s needed and how you like things done, always aiming to make your home feel exactly how you want it — clean, organised, and comfortable. Having help with household tasks can free up your energy and time to focus on the things you enjoy most.

Support Coordination

Support Coordination helps you make sense of your NDIS plan and get the most from your funding. At Reach Assist, we work alongside you to connect with the right providers, organise services, and build a strong, reliable support network. We make the process clear and manageable, helping you set priorities, troubleshoot any issues that come up, and adjust your supports as your needs change. 

Our goal is to help you feel confident and in control, without being overwhelmed by the paperwork and processes. Support Coordination is about giving you real choice, making sure your voice is heard, and ensuring that every service you access truly meets your needs.

Access and Maintain Employment or Higher Education

Finding and keeping a job or pursuing education can open up a world of opportunities — but it can also come with challenges. Reach Assist offers practical support to help you explore employment options or start further study. Development of Daily Living and Life Skills

Building everyday skills can open the door to more independence and confidence. At Reach Assist, our Development of Daily Living and Life Skills programs are tailored to what matters most to you — whether that’s budgeting, cooking, managing personal appointments, shopping independently, or building relationships. 

We work alongside you at your pace, providing practical support and encouragement to build real-world skills that you can use every day. It’s not about ticking off a checklist — it’s about creating routines, habits, and confidence that make life feel more manageable and empowering.
